Responsibilities
- Monitor patient vitals and symptoms via telehealth platforms
- Conduct virtual health assessments and medication reconciliation
- Coordinate care transitions between in-person and virtual settings
- Document electronic health records with precision and timeliness
- Collaborate with specialists using secure communication systems
- Provide health education to patients and families via digital channels
- Participate in rotating on-site clinical rotations (2-3 days/month)
Qualifications
- Associate's or Bachelor's degree in Nursing
- Active Illinois RN license
- Basic Life Support (BLS) certification
- Experience with telehealth platforms (Epic preferred)
- Strong critical thinking and communication skills
- Ability to adapt to rapidly changing healthcare technologies
- Compassion and cultural sensitivity in virtual patient interactions
