Responsibilities
- Provide remote patient assessment and care coordination via telehealth platforms
- Collaborate with interdisciplinary teams to develop and implement patient care plans
- Monitor and document patient progress using electronic health records (EHR)
- Educate patients and families on health management and medication protocols
- Respond to patient inquiries and escalate critical situations appropriately
- Participate in continuous quality improvement initiatives
- Adhere to HIPAA compliance and hospital policies in all remote interactions
Qualifications
- Active RN license in Virginia (or compact state)
- Associate or Bachelor's degree in Nursing (ADN/BSN)
- Recent nursing graduate or less than 1 year of clinical experience
- Basic computer proficiency and familiarity with telehealth systems
- Strong critical thinking and communication skills
- Ability to work independently with minimal supervision
- CPR/BLS certification (or obtain within 30 days)
- Pass background and drug screening
