Key Responsibilities
- Assess patient symptoms and medical history via phone or secure messaging to determine appropriate level of care.
- Triage incoming calls and patient inquiries to prioritize urgent medical needs.
- Document all patient interactions and triage decisions accurately in the Electronic Health Record (EHR).
- Provide health education and self-care instructions to patients and families.
- Collaborate with physicians and other care team members to ensure seamless patient transitions.
- Monitor for changes in patient status and escalate critical issues immediately.
Qualifications
- Graduate of an accredited Nursing Program (LPN, RN, or equivalent).
- No prior clinical experience required; training provided.
- Current, active state nursing license (or eligibility to obtain one).
- Strong verbal and written communication skills.
- Ability to remain calm and composed under pressure.
- Proficiency with basic computer applications and EHR systems.
