Responsibilities
- Conduct comprehensive telephone and in-person patient assessments to determine urgency of care needs
- Provide expert clinical guidance to patients, families, and healthcare providers
- Accurately document patient encounters in EMR systems following HIPAA protocols
- Collaborate with physicians, specialists, and care teams to coordinate patient transitions
- Implement triage protocols for emergency, urgent, and non-urgent cases
- Delicate patient education on treatment plans, home care instructions, and resource navigation
- Participate in continuous quality improvement initiatives for triage services
Qualifications
- Current California RN license with active status
- Minimum 2 years acute care nursing experience
- BSN degree or equivalent certification strongly preferred
- BLS and ACLS certifications required
- Proven expertise in emergency triage protocols and clinical decision-making
- Exceptional communication skills with ability to de-escalate stressful situations
- Proficient in Epic or similar EMR systems
- Ability to work independently with minimal supervision
