Responsibilities
- Conduct comprehensive telephone and digital health assessments to evaluate patient symptoms and medical histories.
- Apply clinical judgment to determine appropriate triage levels, recommending home care, self-care, or immediate emergency services.
- Document patient interactions accurately and securely within the Electronic Health Record (EHR) system.
- Provide clear, empathetic, and educational guidance to patients and caregivers regarding treatment plans and preventative care.
- Collaborate effectively with physicians and other healthcare professionals to ensure continuity of care.
- Maintain current knowledge of medical protocols, pharmacology, and clinical guidelines.
- Escalate critical cases and emergencies immediately to on-call providers or emergency services.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the State of Texas.
- Minimum of 2 years of clinical nursing experience, preferably in a triage, urgent care, or telehealth setting.
- Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications required.
- Strong computer proficiency with experience using EMR/EHR systems.
- Excellent verbal and written communication skills with a focus on patient privacy and confidentiality.
- Ability to remain calm and make sound decisions under pressure in high-stress situations.
- Flexibility to work varying shifts including evenings, weekends, and holidays as needed.
