Responsibilities
- Provide compassionate in-home care to patients across Charlotte metro area
- Assist with ADLs (bathing, dressing, mobility) and vital sign monitoring
- Document care notes electronically via secure mobile platform
- Coordinate care plans with remote nursing supervisors
- Respond to patient inquiries via telehealth portal
- Maintain accurate medication logs remotely
- Participate in virtual care team huddles
Qualifications
- Active NC CNA certification
- Minimum 1 year CNA experience (home care preferred)
- Reliable transportation with valid driver's license
- Proficiency with electronic health records (EHR)
- Strong telehealth communication skills
- COVID-19 vaccination (medical/religious exemptions considered)
- Ability to lift 50 lbs occasionally
- Basic computer skills for remote documentation
