Responsibilities
- Provide compassionate personal care to patients including bathing, dressing, and feeding assistance
- Monitor vital signs and report changes in patient condition to clinical staff
- Assist with mobility transfers and use of adaptive equipment
- Maintain accurate electronic health records (EHR) via our remote portal
- Coordinate care plans with interdisciplinary teams via virtual meetings
- Administer medications under RN supervision (per state guidelines)
- Support patient emotional well-being through active listening and companionship
Qualifications
- Active Texas CNA certification in good standing
- Minimum 1 year experience in long-term care or hospital setting
- Current CPR/BLS certification
- Proficient with EHR systems and telehealth platforms
- Ability to work independently with minimal on-site supervision
- Strong communication skills for remote patient coordination
- Flexibility to work weekends and variable shifts
- Valid Texas driver's license and reliable transportation
