Responsibilities
- Provide compassionate in-home and facility-based patient care including bathing, dressing, and mobility assistance
- Monitor vital signs and document patient observations in our remote health monitoring system
- Assist with medication administration and treatment protocols under RN supervision
- Coordinate telehealth appointments and remote patient check-ins for post-acute care
- Maintain accurate electronic health records with secure remote access
- Collaborate with interdisciplinary teams via virtual care platforms
- Support patient education through remote coaching and digital resources
Qualifications
- Active Ohio CNA certification in good standing
- Minimum 1 year of clinical experience in home health or long-term care
- CPR/BLS certification required
- Proficiency with telehealth technology and EHR systems
- Ability to work flexible hours including weekends and holidays
- Strong communication skills for virtual patient interactions
- Reliable transportation for in-home visits
- Pass background check and drug screening
