Responsibilities
- Deliver personalized care plans including medication management, wound care, and mobility assistance
- Assist with daily living activities (bathing, dressing, grooming) while preserving client dignity
- Monitor vital signs and report health changes to clinical supervisors
- Prepare nutritious meals and perform light housekeeping duties
- Transport clients to medical appointments and social activities
- Maintain accurate care documentation and client records
- Provide companionship and emotional support to enhance client well-being
Qualifications
- Minimum 2 years of professional home health or hospice experience
- Current Texas HHA certification or CNA license
- Valid driver’s license with clean driving record
- Reliable transportation and willingness to travel throughout Plano area
- CPR/First Aid certification (or willingness to obtain)
- Excellent communication and interpersonal skills
- Ability to lift 50+ pounds and perform physical care tasks
