We are looking for a professional who can effectively assess patient needs, make critical decisions, and provide exceptional service in a fast-paced environment.
Responsibilities
- Assess & Triage: Evaluate patient symptoms via phone, secure messaging, or portal to determine the appropriate level of care and urgency.
- Care Coordination: Facilitate referrals to appropriate specialists, primary care physicians, or emergency services based on clinical judgment.
- Documentation: Maintain accurate, detailed, and timely records of all patient interactions within the Electronic Health Record (EHR).
- Patient Education: Provide clear instructions on medication management, follow-up care, and lifestyle modifications to patients and families.
- Protocol Adherence: Ensure all clinical decisions align with Apex Health Systems protocols and industry standards.
- Collaboration: Work closely with physicians, care managers, and other healthcare professionals to ensure seamless patient transitions.
Qualifications
- License: Current, active Registered Nurse (RN) license in New York State.
- Certifications: BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 3-5 years of clinical experience in acute care, emergency room, or triage nursing preferred.
- Communication: Exceptional verbal and written communication skills with the ability to convey complex medical information clearly.
- Technology: Proficiency with Electronic Health Records (EHR) and telehealth platforms.
- Professionalism: Strong ability to remain calm under pressure and demonstrate empathy in high-stress situations.
