Responsibilities
- Assist patients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and report changes in patient health and behavior to the nursing team.
- Prepare nutritious meals and assist with feeding if necessary.
- Perform light housekeeping tasks such as laundry, cleaning, and changing linens.
- Provide companionship and emotional support to patients to reduce isolation.
- Accompany patients to medical appointments and therapy sessions.
- Administer medication reminders as directed by the RN/Case Manager.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) license required.
- Current CPR and First Aid certification.
- Strong interpersonal and communication skills.
- Ability to lift and move patients (weight up to 50 lbs) safely.
- Valid California Driver’s License and reliable transportation.
- Pass a background check and drug screening.
- Previous experience in home health or caregiving is preferred but not mandatory.
