Responsibilities

  • Personal Care Assistance: Assist patients with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and mobility support.
  • Medication Management: Remind patients to take prescribed medications on time and report any adverse reactions or missed doses to the nursing supervisor immediately.
  • Companionship & Engagement: Provide emotional support, engage patients in conversation, and facilitate social activities to reduce isolation and improve mental well-being.
  • Vital Signs Monitoring: Accurately measure and record patient vital signs such as blood pressure, pulse, respiration rate, and temperature.
  • Household Support: Assist with light housekeeping duties, meal preparation, and laundry to ensure a safe and clean living environment for the patient.
  • Transportation & Errands: Accompany patients to medical appointments, therapy sessions, and social outings, or run errands as requested.
  • Documentation: Maintain accurate, up-to-date, and timely clinical documentation in the electronic health record system.

Qualifications

  • Licensure: Active and valid Texas Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license required.
  • Certifications: Current Basic Life Support (BLS) and CPR certification.
  • Experience: Previous experience in home health, long-term care, or geriatric care is highly preferred.
  • Communication: Strong verbal and written communication skills with the ability to interact professionally with patients, families, and medical staff.
  • Physical Requirements: Ability to lift, bend, stand, and walk for extended periods; ability to assist with patient transfers.
  • Driving: Valid driver's license with a clean driving record and reliable personal transportation.
