Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide assistance with mobility and transfers to ensure patient safety.
- Monitor and record vital signs and report any changes in the patient's condition to the registered nurse.
- Prepare meals according to prescribed diets and assist with light housekeeping related to patient care.
- Medication reminders and adherence support.
- Offer emotional support and companionship to patients and their families.
- Transport patients to medical appointments and therapy sessions as needed.
Qualifications
- High school diploma or GED equivalent.
- Current Illinois Home Health Aide (HHA) or Personal Care Attendant (PCA) certification.
- Current Basic Life Support (BLS) and CPR certification.
- Valid Driver's License and reliable transportation.
- Strong communication skills and a patient-centered approach.
- Ability to work independently and as part of a multidisciplinary team.
- Pass a background check and drug screen.
