Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Monitor vital signs, medication schedules, and general health status; report any significant changes to the nursing team immediately.
- Provide emotional support and companionship to reduce isolation and improve the patient's quality of life.
- Prepare nutritious meals according to dietary restrictions and assist with light housekeeping tasks.
- Assist with mobility, transfers, and ambulation using proper body mechanics and equipment.
- Accompany clients to medical appointments, physical therapy sessions, and social activities.
- Document daily care activities and progress in patient records accurately and timely.
Qualifications
- Valid Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification required.
- Current Basic Life Support (BLS) and CPR certification.
- Valid driver's license and reliable transportation (may be required for home visits).
- Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare professionals.
- Physical stamina to lift, bend, stand, and walk for extended periods.
- Must pass a comprehensive background check and drug screening.
