Responsibilities
- Provide essential daily living assistance, including bathing, dressing, grooming, and toileting.
- Monitor patient health status and report any changes or concerns to the nursing team immediately.
- Assist with light housekeeping, laundry, and meal preparation according to dietary restrictions.
- Administer prescribed medications and treatments under the supervision of a licensed nurse.
- Accompany patients to medical appointments, therapy sessions, and social activities.
- Document patient care activities and progress notes accurately and in a timely manner.
- Offer emotional support and companionship to improve the patient's quality of life.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification is required.
- Current, valid driver's license and reliable transportation are mandatory.
- Ability to pass a comprehensive background check and drug screen.
- Strong interpersonal skills with the ability to communicate effectively with patients and families.
- Physical stamina to perform lifting, bending, and standing for extended periods.
- Willingness to travel within the Madison area to client homes.
