Responsibilities
- Assist clients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting.
- Utilize provided mobility equipment (lifts, patient lifts, and hoists) to safely transfer and move patients.
- Monitor patient health status, reporting any changes or concerns to the Registered Nurse or supervisor immediately.
- Prepare nutritious meals and assist with feeding according to dietary restrictions.
- Perform light housekeeping and laundry tasks to maintain a safe and hygienic living environment.
- Provide medication reminders and document administration accurately.
- Communicate effectively with patients, families, and the interdisciplinary care team.
Qualifications
- Current and valid California Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification.
- Valid California driver’s license and a clean driving record.
- Ability to lift, push, and pull up to 50 lbs using provided mechanical assistance.
- CPR/BLS (Basic Life Support) certification is strongly preferred.
- Strong interpersonal and communication skills.
- Basic computer skills for electronic documentation.
