Responsibilities
- Provide personal care assistance, including bathing, grooming, and dressing support for patients.
- Assist with light housekeeping tasks and laundry specifically related to patient care.
- Monitor and accurately record vital signs, daily health status, and any changes in behavior.
- Prepare nutritious meals and assist with feeding if necessary.
- Accompany clients to medical appointments, therapy sessions, and social activities.
- Offer companionship and emotional support to reduce isolation and improve mental well-being.
- Administer medication reminders as directed by the Registered Nurse case manager.
Qualifications
- High School Diploma or GED is required.
- Current Oregon Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification is preferred (training provided for self-motivated candidates).
- CPR and Basic Life Support (BLS) certification preferred or willing to obtain.
- Valid driver's license and reliable transportation are required.
- Strong communication and interpersonal skills with a patient demeanor.
- Ability to pass a background check and drug screening.
- Physical ability to assist patients with mobility and transfers.
