Responsibilities
- Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide medication reminders and assist with light medication administration as directed by the RN.
- Prepare nutritious meals and assist with meal planning based on dietary restrictions.
- Support mobility by helping clients walk, transfer, or use wheelchairs safely.
- Accompany clients to medical appointments, grocery stores, and social activities.
- Perform light housekeeping and laundry duties to ensure a safe and clean living environment.
- Document daily care logs and communicate any changes in the client's condition to the supervising nurse or case manager.
Qualifications
- High school diploma or GED is preferred.
- Certified Nursing Assistant (CNA) or Home Health Aide (HHA) certification is highly preferred.
- Current Wisconsin State Driver's License and reliable transportation.
- Ability to pass a background check and drug screen.
- Strong interpersonal communication skills and a patient demeanor.
- Physical ability to lift, bend, and stand for extended periods.
- CPR and First Aid certification is a plus.
