Responsibilities
- Conduct comprehensive virtual patient assessments and health screenings via telehealth platform
- Develop and implement personalized care plans for chronic disease management
- Coordinate care transitions between facilities and community resources
- Provide patient education on medication adherence and lifestyle modifications
- Document clinical encounters accurately in electronic health records (EHR)
- Collaborate with specialists and primary care providers for integrated care
- Respond to urgent patient inquiries during weekend shifts
- Participate in continuous quality improvement initiatives
Qualifications
- Active RN license in Florida (FL RN)
- BSN degree (or equivalent experience)
- Minimum 2 years clinical nursing experience
- Telehealth or remote care certification preferred
- Strong proficiency in EHR systems and telehealth platforms
- Excellent communication and critical thinking skills
- Ability to work independently during weekend shifts
- Current BLS/CPR certification
