Responsibilities
- Personal Care Assistance: Assist clients with Activities of Daily Living (ADLs) including bathing, grooming, dressing, and toileting with the utmost dignity and respect.
- Medication Management: Remind clients to take their prescribed medications on time and report any changes in health status to the RN case manager immediately.
- Companionship: Provide emotional support and engage clients in meaningful conversations, hobbies, and recreational activities to enhance their quality of life.
- Household Support: Perform light housekeeping duties such as laundry, meal preparation, and light cleaning to maintain a safe and sanitary living environment.
- Vital Signs Monitoring: Monitor and record vital signs, including weight, temperature, and blood pressure, as directed by the healthcare team.
- Transportation Assistance: Provide safe transportation to and from medical appointments or errands as needed.
- Documentation: Maintain accurate and timely documentation of care provided in the electronic health record system.
Qualifications
- Education: High School Diploma or GED equivalent.
- Certification: Current California State Home Health Aide (HHA) certification and valid CPR/BLS certification.
- Driver's License: Valid California Driver's License and reliable transportation (a valid ID and proof of insurance may be required).
- Experience: Prior experience in home health, nursing, or caregiving is preferred but not always required for exceptional candidates.
- Communication: Excellent verbal and written communication skills with the ability to interact effectively with patients, families, and medical staff.
- Compassion: A genuine passion for helping others and the ability to demonstrate patience, empathy, and respect.
- Background Check: Ability to pass a background check and drug screening.
