Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide companionship and emotional support to clients to improve their quality of life.
- Prepare nutritious meals and assist with light housekeeping and laundry as needed.
- Monitor and report changes in patient mood, behavior, or physical condition to the RN case manager.
- Assist with mobility, transferring patients to and from bed or wheelchair using proper body mechanics.
- Accompany patients to medical appointments and assist with transportation logistics.
Qualifications
- Current Indiana CNA (Certified Nursing Assistant) or HHA (Home Health Aide) certification required.
- Must possess a valid Indiana Driver's License and reliable transportation.
- Completion of a state-approved Home Health Aide training program.
- Strong interpersonal and communication skills with a patient-centered approach.
- Ability to pass a background check and drug screen.
- CPR and First Aid certification preferred.
