Responsibilities
- Assist clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Monitor client health status and promptly report any changes or concerns to the RN or Case Manager.
- Prepare nutritious meals and assist with light housekeeping and laundry as directed by the care plan.
- Provide companionship and emotional support to ensure clients feel comfortable and engaged.
- Transport clients to medical appointments, therapy sessions, or social activities using personal vehicle.
- Administer medication reminders according to strict physician orders.
- Maintain accurate, up-to-date documentation of care provided.
Qualifications
- High School Diploma or GED required; CNA or HHA certification preferred.
- Current, valid Driver’s License and proof of auto insurance.
- CPR and Basic Life Support (BLS) certification (or willingness to obtain).
- Strong communication skills and the ability to work effectively in a team.
- Ability to pass a comprehensive background check and drug screening.
- Compassionate nature with a genuine desire to help the elderly and disabled.
