Responsibilities
- Provide personal care assistance including bathing, dressing, grooming, and toileting.
- Assist with mobility, transfers, and ambulation using proper body mechanics and patient handling techniques.
- Monitor and record vital signs, weight, and patient health status accurately.
- Prepare nutritious meals and assist with light housekeeping duties to maintain a safe environment.
- Accompany patients to medical appointments, therapy sessions, and social activities.
- Administer prescribed medications as directed by the registered nurse or physician.
- Offer emotional support, companionship, and communication to patients and their families.
- Document daily care logs and report any changes in patient condition to the care coordinator.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) certification required.
- Current Basic Life Support (BLS) and CPR certification from the American Heart Association.
- Valid driver's license and reliable personal transportation.
- Ability to pass a comprehensive background check and drug screening.
- Strong interpersonal and communication skills with a patient demeanor.
- Experience with elderly or disabled individuals is highly preferred.
- Flexibility to work varying shifts, including weekends, nights, and holidays.
