Responsibilities
- Assist clients with Activities of Daily Living (ADLs) including bathing, dressing, grooming, and toileting.
- Monitor and record vital signs and report any changes in client condition to the nursing supervisor immediately.
- Prepare nutritious meals and assist with light housekeeping and laundry as assigned.
- Provide medication reminders and assist with medication administration under the supervision of a nurse.
- Accompany clients to medical appointments, therapy sessions, and social activities.
- Offer emotional support and companionship to reduce isolation and improve quality of life.
- Maintain accurate documentation of care provided in accordance with agency policies.
Qualifications
- Valid Wisconsin Home Health Aide (HHA) certification or CNA license required.
- Current Basic Life Support (BLS) certification is strongly preferred.
- High School Diploma or GED equivalent.
- Previous experience in home care, nursing, or caregiving is preferred but not mandatory.
- Strong interpersonal and communication skills.
- Ability to lift, bend, and stand for extended periods as required by the job.
- Valid driver's license and reliable transportation.
