Responsibilities
- Provide daily personal care assistance, including bathing, grooming, and dressing.
- Assist with activities of daily living (ADLs) such as eating, toileting, and mobility.
- Monitor and report changes in patient condition to the supervising nurse or case manager.
- Administer medications and reminders as per the patient's care plan.
- Prepare nutritious meals and assist with light housekeeping to maintain a safe environment.
- Offer companionship and emotional support to enhance the patient's quality of life.
- Document care services and observations accurately in electronic health records.
Qualifications
- Valid High School Diploma or GED required.
- Certified Home Health Aide (CHHA) certification is strongly preferred.
- Current Basic Life Support (BLS) and CPR certification.
- Valid Arizona driver's license and reliable transportation.
- Ability to pass a comprehensive background check and drug screening.
- Strong communication skills and a genuine desire to help others.
- Physical stamina to stand, walk, and lift patients as needed.
