Responsibilities
- Assist patients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting.
- Provide medication reminders and assist with light housekeeping and meal preparation as assigned.
- Monitor patient health and report any changes in physical or mental condition to the Registered Nurse (RN) or Case Manager.
- Accompany patients to medical appointments and provide transportation support.
- Foster a warm, supportive, and safe environment to promote emotional well-being.
- Document daily care logs and care plans accurately and in a timely manner.
- Adhere to all state and federal regulations regarding patient privacy and safety.
Qualifications
- High School Diploma or GED required.
- Certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) preferred.
- Current CPR and First Aid certification (or willingness to obtain).
- Valid Driver’s License and reliable transportation.
- Excellent communication skills and a genuine passion for helping others.
- Ability to pass a comprehensive background check and drug screening.
- Strong physical stamina and the ability to lift and assist patients as needed.
