Responsibilities
- Provide direct personal care assistance to clients with Activities of Daily Living (ADLs), including bathing, dressing, grooming, and toileting.
- Assist with mobility, transfers, and the use of mobility aids (wheelchairs, walkers).
- Monitor and record vital signs and report any changes in the client's condition to the supervising nurse or case manager.
- Prepare nutritious meals and assist with light housekeeping and laundry as assigned.
- Offer companionship and emotional support to clients, engaging in conversation and recreational activities.
- Administer medications as directed by the case plan and licensed nurse.
- Accompany clients to medical appointments and assist with transportation logistics.
Qualifications
- Certified Home Health Aide (CHHA) or Certified Nursing Assistant (CNA) license required.
- Current Basic Life Support (BLS) certification.
- Valid California Driver's License and current auto insurance.
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a healthcare team.
- Physical stamina to perform lifting and assisting with mobility tasks.
- Completion of a background check and drug screen.
