Responsibilities
- Provide remote patient coordination and care scheduling via telehealth platforms
- Assist patients with daily activities (bathing, dressing, mobility) at partner facilities
- Monitor vital signs and document patient health records remotely
- Collaborate with RNs and healthcare teams via digital communication tools
- Support medication administration under supervision at on-site locations
- Transport patients to appointments as needed
- Maintain HIPAA-compliant digital documentation systems
Qualifications
- No prior healthcare experience required – training provided!
- High school diploma or equivalent (GED)
- Basic computer skills for remote work (email, scheduling software)
- Valid Pennsylvania driver's license (for patient transport)
- Ability to lift 50+ pounds and stand for extended periods
- Compassionate attitude and strong communication skills
