Responsibilities
- Conduct comprehensive phone and video triage assessments for patients presenting a variety of medical symptoms.
- Utilize clinical decision-making skills to prioritize patient needs and determine the appropriate level of care (urgent vs. non-urgent).
- Facilitate seamless referrals to physicians, specialists, or emergency services when necessary.
- Document all patient interactions, assessments, and care plans accurately in the Electronic Health Record (EHR).
- Provide health education and support to patients and their families regarding chronic disease management and preventive care.
- Collaborate with the nursing and medical leadership team to continuously improve triage protocols.
- Manage high call volumes while maintaining a calm, professional, and empathetic demeanor.
Qualifications
- Current, unencumbered Registered Nurse (RN) license in the state of Minnesota.
- Completion of a paid training program is preferred, but we are willing to train the right candidate.
- Minimum of 1-2 years of nursing experience in a clinical or call center setting (preferred).
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Strong computer proficiency with EMR systems and the ability to learn new software quickly.
- Excellent verbal communication skills with the ability to de-escalate stressful situations.
- Strong analytical and critical thinking skills with a high attention to detail.
