Responsibilities
- Provide direct personal care to patients, including assistance with bathing, dressing, grooming, and toileting.
- Monitor and accurately record patient vital signs, such as blood pressure, temperature, pulse, and respiration rates.
- Assist with patient mobility and transfers, strictly adhering to safety protocols to prevent falls.
- Prepare and serve meals according to individual dietary restrictions and preferences.
- Document daily care activities, changes in patient condition, and any incidents in the electronic health record.
- Communicate effectively with the nursing team, physicians, and family members regarding patient needs and updates.
- Maintain a clean, sanitized, and organized patient environment to ensure infection control standards.
Qualifications
- Current and valid California CNA certification (State Tested Nursing Assistant - CST).
- Previous experience in a home health, hospice, or long-term care setting is preferred.
- Strong interpersonal and communication skills with the ability to interact compassionately with patients and families.
- Ability to perform physical tasks, including lifting and transferring patients up to 50 lbs safely.
- Valid driver's license and proof of auto insurance (for travel to client homes if required).
- CPR and Basic Life Support (BLS) certification is highly desired.
- Reliable, punctual, and detail-oriented work ethic.
