Responsibilities
- Provide direct patient care, including bathing, dressing, and grooming assistance, ensuring patient comfort and hygiene.
- Monitor and record vital signs, such as blood pressure, temperature, and pulse, reporting any abnormalities to the nursing staff immediately.
- Assist patients with mobility, including transferring them to and from beds, wheelchairs, and using walkers.
- Prepare and serve meals according to dietary restrictions, ensuring patients have adequate nutrition and hydration.
- Document patient care activities, moods, and behaviors accurately in electronic health records (EHR) systems.
- Communicate effectively with patients, families, and the interdisciplinary care team to ensure continuity of care.
- Maintain a clean, safe, and organized environment in patient rooms and common areas.
Qualifications
- Current and active Certified Nursing Assistant (CNA) license in the state of Arizona.
- High school diploma or GED equivalent required.
- CPR/BLS (Basic Life Support) certification must be current and valid.
- Strong interpersonal and communication skills with a focus on patient-centered care.
- Ability to stand for extended periods and lift patients safely within weight limits.
- Flexibility to work varying shifts, including mornings, evenings, and weekends.
- Previous experience in a home health or long-term care setting is preferred but not required.
