Responsibilities
- Provide virtual patient support via telehealth platforms
- Assess patient needs and coordinate care plans with clinical teams
- Document patient interactions accurately in our EHR system
- Conduct wellness checks and health education remotely
- Collaborate with RNs and physicians to optimize patient outcomes
- Maintain HIPAA compliance in all digital communications
- Participate in virtual training and continuous improvement initiatives
Qualifications
- Active California CNA certification required
- Minimum 1 year of CNA experience in home care or clinical setting
- Proficiency with telehealth platforms and electronic health records
- Strong communication skills and empathy for diverse patient populations
- Ability to work independently with minimal supervision
- Valid CPR certification
- Clear background check and TB test
- Reliable internet connection and quiet home workspace
