Responsibilities
- Provide Personal Care: Assist clients with activities of daily living (ADLs) including bathing, dressing, grooming, and toileting in a respectful and professional manner.
- Monitor Health: Track vital signs, report changes in condition, and communicate effectively with the Registered Nurse (RN) Case Manager regarding patient needs.
- Medication Management: Remind clients to take prescribed medications and assist with medication administration as directed by the supervising nurse.
- Companionship: Engage clients in social activities, conversation, and hobbies to promote mental and emotional well-being.
- Household Support: Assist with light housekeeping duties such as laundry, meal preparation, and errands to ensure a safe and clean living environment.
- Transportation: Provide safe transportation to and from medical appointments or social activities when needed.
Qualifications
- Certification: Current, valid California Home Health Aide (HHA) or Certified Nursing Assistant (CNA) license.
- Experience: Previous experience in home health or long-term care is preferred but not always required.
- Driverβs License: A valid driver's license, reliable transportation, and current auto insurance are required.
- Communication: Excellent verbal and written communication skills with the ability to follow instructions accurately.
- Compassion: A genuine, caring demeanor and the ability to build trust with patients and families.
- Background Check: Ability to pass a background check and drug screen.
