Responsibilities
- Provide direct patient care in acute and ambulatory settings with focus on chronic disease management
- Administer medications, perform wound care, and conduct patient assessments per clinical protocols
- Collaborate with interdisciplinary teams to develop and implement individualized care plans
- Document patient encounters accurately in EMR systems ensuring HIPAA compliance
- Educate patients and families on health maintenance and preventive care strategies
- Participate in quality improvement initiatives and maintain regulatory compliance
Qualifications
- Active California RN license (BRN) in good standing
- Minimum 1 year recent acute care or ambulatory nursing experience
- BLS/CPR certification required; ACLS preferred
- Proficiency in electronic medical records (Epic/EMR systems)
- Strong assessment skills and clinical judgment abilities
- Excellent communication and interpersonal skills
- Ability to work independently and in collaborative team settings
