Responsibilities
- Conduct virtual patient assessments and telehealth consultations via secure HIPAA-compliant platforms
- Develop and implement personalized care plans for chronic disease management

- Document patient encounters accurately in EHR systems within 24-hour turnaround
- Provide health education and medication management support via digital channels

Qualifications
- Active RN license in Minnesota (compact license holders encouraged)
- Minimum 2 years clinical experience in telehealth or ambulatory care
- BLS/CPR certification (ACLS preferred)

- Exceptional communication skills with digital fluency
- Ability to maintain clinical judgment in virtual settings
