Responsibilities
- Conduct virtual patient assessments and triage via secure telehealth platform
- Provide evidence-based nursing interventions and patient education remotely
- Collaborate with multidisciplinary care teams for coordinated treatment plans
- Document patient encounters accurately in EHR systems while maintaining HIPAA compliance
- Monitor and manage chronic conditions through virtual follow-ups
- Respond to urgent patient inquiries and escalate critical situations appropriately
- Participate in continuous quality improvement initiatives for telehealth services
Qualifications
- Active Illinois RN license with unrestricted privileges
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience preferred
- Strong proficiency with EHR systems and telehealth technology
- Exceptional communication and critical thinking skills
- BLS certification required, ACLS preferred
- Ability to work independently with minimal supervision
- Commitment to patient-centered care and cultural competency
