Responsibilities
- Provide comprehensive patient assessment and care coordination via telehealth platforms and in-person visits
- Administer medications and treatments with precision while maintaining accurate electronic health records
- Collaborate with multidisciplinary teams to develop personalized care plans
- Educate patients and families on health management and preventive care
- Monitor patient progress and adjust interventions based on clinical outcomes
- Adhere to HIPAA compliance protocols for all remote communications
- Participate in quality improvement initiatives and peer review processes
Qualifications
- Active Oregon RN license with at least 2 years of clinical experience
- BSN degree strongly preferred; ADN candidates with relevant experience considered
- Proficiency in telehealth platforms (e.g., Teladoc, Amwell)
- Strong proficiency in EHR systems (Epic, Cerner, or similar)
- Certification in BLS and ACLS required
- Exceptional critical thinking and decision-making abilities
- Proven ability to work independently in remote settings
- Excellent communication skills with diverse patient populations
