Responsibilities
- Conduct comprehensive telephone assessments to determine patient urgency and care needs
- Provide accurate clinical advice per established protocols and physician guidelines
- Document detailed patient encounters in EMR systems with precision and timeliness
- Coordinate care transitions between patients, providers, and ancillary services
- Educate patients on symptoms, home care, and when to seek emergency care
- Maintain HIPAA compliance and strict confidentiality standards
- Collaborate with multidisciplinary teams for optimal patient outcomes
Qualifications
- Active California RN license in good standing
- Minimum 2 years clinical nursing experience with triage/call center background
- ACLS/BLS certification required; TNCC preferred
- Proficiency in Epic or EMR documentation systems
