Responsibilities
- Conduct Patient Triage: Utilize advanced clinical judgment to assess patient symptoms via phone or secure messaging, determining the urgency of care and appropriate next steps.
- Medical Decision Making: Provide evidence-based nursing interventions, including symptom management advice, medication guidance, and instructions for non-emergency conditions.
- Documentation: Accurately and timely document all patient interactions, assessments, and care plans within our Electronic Health Records (EHR) system.
- Escalation Management: Identify and promptly escalate critical, life-threatening cases to on-call physicians or emergency services to ensure patient safety.
- Patient Education: Educate patients and families on disease prevention, self-care strategies, and available community resources.
- Policy Adherence: Ensure all clinical practices comply with state regulations, HIPAA guidelines, and Central Valley Health Partners protocols.
Qualifications
- Licensure: Current, active Registered Nurse (RN) license in the State of California.
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Experience: Minimum of 2-3 years of clinical nursing experience, preferably in a triage, urgent care, or telehealth setting.
- Communication Skills: Exceptional verbal and written communication skills with the ability to convey complex medical information clearly and empathetically.
- Technology Proficiency: Comfortable navigating electronic health record systems and telehealth platforms.
- Stress Management: Ability to remain calm and make sound decisions under pressure in a high-volume environment.
