Responsibilities
- Conduct thorough patient assessments via phone, secure messaging, or video consultation to determine medical needs and urgency.
- Provide evidence-based triage instructions, including home care remedies and medication guidance.
- Route patient inquiries to the appropriate medical providers, specialists, or emergency services when necessary.
- Accurately document all patient interactions, assessments, and care plans within the Electronic Health Record (EHR) system.
- Collaborate with physicians and nursing staff to ensure continuity of care and follow-up.
- Manage high call volumes while maintaining a calm, professional demeanor and empathy for patients.
Qualifications
- Active Registered Nurse (RN) license in the State of Colorado (or compact state).
- Certifications: Current BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support).
- Minimum of 2-3 years of nursing experience, with specific experience in triage or telehealth preferred.
- Strong interpersonal and communication skills with a focus on patient education.
- Proficiency with Electronic Health Records (EHR) systems (e.g., Epic, Cerner).
- Ability to work independently while adhering to strict clinical protocols and guidelines.
