Responsibilities
- Conduct Patient Triage: Assess patient symptoms via phone or secure messaging to determine the appropriate level of care and urgency.
- Provide Medical Guidance: Offer evidence-based nursing advice, symptom management strategies, and instructions for home care.
- Facilitate Referrals: Coordinate seamless transitions to appropriate healthcare providers, specialists, or urgent care facilities when necessary.
- Documentation: Maintain accurate and comprehensive electronic health records (EHR) of all patient interactions and triage decisions.
- Monitor Patient Status: Follow up with patients to ensure their conditions are stable and to provide additional support as needed.
- Collaborate with Providers: Communicate effectively with on-site physicians and nurses to ensure continuity of care.
Qualifications
- Licensure: Active Registered Nurse (RN) license in the state of Florida.
- Certifications: Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Experience: Minimum of 2-3 years of nursing experience, preferably in a triage, urgent care, or telehealth setting.
- Skills: Strong clinical assessment skills and the ability to think critically under pressure.
- Communication: Excellent verbal and written communication skills with a patient-centered approach.
- Technology: Proficiency with Electronic Health Records (EHR) systems and medical software.
