Responsibilities
- Assess patient symptoms via phone triage to determine urgency and appropriate care pathways
- Collaborate with physicians and specialists to develop care plans
- Maintain accurate electronic health records (EHR) documentation
- Provide compassionate patient education on home care and follow-up protocols
- Coordinate with emergency services for critical patient transfers
- Monitor patient progress and adjust care plans as needed
- Participate in continuous quality improvement initiatives
Qualifications
- Current Florida RN license with minimum 2 years emergency/clinical experience
- BLS and ACLS certifications required
- Strong assessment and critical thinking skills
- Excellent communication and de-escalation abilities
- Proficiency in EHR systems (Epic preferred)
- Ability to work independently with sound judgment
- Flexibility to work evenings, weekends, and holidays
- BSN degree preferred
