Responsibilities
- Provide high-quality, compassionate direct patient care in the home or facility setting, including assessments, medication administration, and vital signs monitoring.
- Develop and implement individualized nursing care plans based on patient needs and physician orders.
- Educate patients and their families on disease prevention, health maintenance, and care instructions.
- Collaborate effectively with physicians, case managers, and other healthcare professionals to ensure holistic patient outcomes.
- Document all patient interactions, observations, and care provided in compliance with HIPAA and agency policies.
- Respond promptly to patient calls and emergencies with a focus on safety and comfort.
Qualifications
- Current, unencumbered Michigan Registered Nurse (RN) license.
- BLS (Basic Life Support) and ACLS (Advanced Cardiovascular Life Support) certifications required.
- Minimum of 2 years of professional nursing experience preferred, with home health or critical care experience a plus.
- Strong clinical assessment skills and ability to prioritize multiple tasks in a fast-paced environment.
- Excellent communication and interpersonal skills with a patient-centered approach.
- Valid driver's license and reliable transportation.
