Responsibilities
- Conduct virtual patient assessments and monitor health status through telehealth platforms
- Develop and implement individualized care plans for diverse patient populations
- Collaborate with physicians and specialists to coordinate patient care remotely
- Provide patient education and health coaching via secure video and messaging
- Document clinical encounters accurately in electronic health records (EHR)
- Respond to patient inquiries and triage urgent situations appropriately
- Participate in quality improvement initiatives and telehealth protocol development
Qualifications
- Active California Registered Nurse (RN) license required
- Bachelor of Science in Nursing (BSN) preferred
- Minimum 2 years of clinical nursing experience
- Telehealth or virtual care experience highly desirable
- Strong assessment and critical thinking skills
- Proficiency with EHR systems and telehealth technology
- Excellent communication and interpersonal abilities
- CPR/BLS certification maintained
