Responsibilities
- Deliver high-quality patient care through telehealth platforms and in-clinic visits
- Collaborate with multidisciplinary teams to develop personalized care plans
- Utilize electronic health records to document patient interactions accurately
- Provide health education and coaching to patients remotely
- Monitor patient vitals and symptoms via remote monitoring technology
- Participate in quality improvement initiatives and evidence-based practice
- Maintain compliance with HIPAA and healthcare regulations
Qualifications
- Active RN license in Indiana (or compact state)
- Minimum 2 years of clinical nursing experience
- BSN preferred (ADN candidates with experience considered)
- Experience with telehealth platforms (e.g., Teladoc, Amwell)
- BLS/CPR certification required (ACLS preferred)
- Strong assessment and critical thinking skills
- Exceptional communication and technological proficiency
- Ability to work independently in remote settings
