Responsibilities
- Conduct comprehensive virtual patient assessments and develop personalized care plans
- Provide telehealth consultations and medication management support
- Collaborate with interdisciplinary teams to coordinate patient care
- Monitor and document patient progress using EHR systems
- Educate patients on chronic disease management and preventive care
- Respond to urgent patient inquiries and escalate critical situations
- Participate in quality improvement initiatives and clinical research
Qualifications
- Active RN license in Minnesota
- BSN degree (ADN candidates with 3+ years experience considered)
- Minimum 2 years clinical nursing experience
- Proficiency with telehealth platforms and EHR systems
- Strong critical thinking and decision-making skills
- Excellent communication abilities for virtual patient interactions
- Ability to maintain HIPAA compliance in remote settings
- Certification in BLS and ACLS required
