Responsibilities
- Conduct comprehensive patient assessments and triage via secure telehealth platforms.
- Develop and implement individualized care plans in collaboration with physicians and care teams.
- Document all patient interactions, assessments, and interventions accurately in electronic health records (EHR).
- Monitor patient conditions and escalates critical issues immediately to on-call providers.
- Provide patient education and discharge planning instructions via virtual channels.
- Collaborate with multidisciplinary teams to ensure seamless continuity of care.
Qualifications
- Active and unencumbered Registered Nurse (RN) license in the state of Colorado.
- Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications.
- Minimum of 1-2 years of nursing experience, preferably in a remote or telehealth setting.
- Strong computer proficiency with experience using EMR/EHR systems (e.g., Epic, Cerner).
- Excellent verbal and written communication skills with a focus on patient empathy.
- Ability to work independently with high attention to detail and confidentiality.
