Responsibilities
- Conduct comprehensive patient assessments and triage via telehealth platforms.
- Develop and implement individualized patient care plans based on evidence-based practice.
- Administer medications and treatments in accordance with state regulations and provider orders.
- Collaborate with physicians and interdisciplinary care teams to ensure continuity of care.
- Maintain accurate and timely documentation in electronic health records (EHR).
- Provide patient education and support regarding disease management and health promotion.
Qualifications
- Current, unencumbered RN license in the State of Ohio.
- Bachelor of Science in Nursing (BSN) preferred; Associate degree accepted.
- Minimum of 2 years of clinical nursing experience (preferred).
- Proficiency with electronic health records (EHR) and telehealth technology.
- Strong communication and interpersonal skills.
- CPR/BLS certification required.
