Responsibilities
- Provide telehealth consultations and remote patient monitoring for diverse populations
- Collaborate with multidisciplinary teams to develop personalized care plans
- Document patient encounters accurately in electronic health records
- Coordinate care transitions between hospital, outpatient, and home settings
- Participate in continuous quality improvement initiatives
- Maintain HIPAA compliance in all digital communications
- Educate patients on chronic disease management and preventive care
Qualifications
- Active RN license in Massachusetts (required)
- Bachelor of Science in Nursing (BSN) preferred
- 2+ years acute care or telehealth experience
- Proficiency with telehealth platforms and EHR systems
- Strong critical thinking and problem-solving abilities
- Excellent communication skills for virtual interactions
- Certification in Basic Life Support (BLS)
- Ability to work independently with minimal supervision
