Responsibilities
- Provide virtual patient assessments and develop personalized care plans
- Monitor patient progress through telehealth platforms and EMR systems
- Coordinate care with interdisciplinary teams for optimal outcomes
- Delicate patient education on chronic conditions and treatment protocols
- Respond to urgent inquiries and triage critical situations remotely
- Maintain accurate documentation in Epic EMR systems
- Participate in virtual case conferences and quality improvement initiatives
Qualifications
- Active RN license in Washington State
- BSN degree (ADN candidates with 5+ years experience considered)
- Minimum 2 years clinical nursing experience
- Telehealth or remote nursing experience preferred
- Proficiency with Epic EMR and telehealth technologies
- Strong critical thinking and assessment skills
- Ability to work independently with minimal supervision
- Certification in Basic Life Support (BLS)
